Developer the DarkProgramer has released a beta of his Trophy Unlocker application. If you have a hacked PS4, this Windows utility will allow you to patch a game prior to installing it, in order to unlock its trophy. From the developer’s readme:
Run TrophyUnlocker.exe
Add the Param.sfo of the current game you want to run.
Add the Nptitle and NpBlind files add the trophy.trp file
All can be dumped with the app dump payload.Click build.
Now install the created pkg on the ps4
(If the game is installed it will overwrite it so be careful)
Run it.
Click the arrows up and down to select the trophy you want unlocked
[ ] to view the trophy list.
X to unlock.

Be warned that this apparently generates a new package for you to install, and will overwrite your previous installation of the game if you already had it installed.
Early testers of Trophy Unlocker are reporting mixed results, with some people stating that the generated package returns an error (Failed to start the app (NP-34942-2)) when running. Others are saying this is working as expected.

Trophy Unlocker is open source, so you can have a look at what it does inside, although to be perfectly honest, the source code is a mix of all of the developer’s projects in a single repository, and is a bit messy.
We haven’t tested the app beyond confirming the GUI actually runs on Windows, so please run at your own risk.
